Am I better off choosing contempt of the family court orders

I have been diligently paying court ordered alimony for 15 years. I have a medical crisis and subsequent financial crisis. I had to quit a good job to attend to this crisis and I do not have any income to support my Ex. My Ex refuses to renegotiate and I cannot afford to go to court to attempt a never before granted modification of alimony.
